Proposal Forms

STOP! READ THIS FIRST As of March 2016, all course proposals may be submitted via the new online course workflow system, in place of the committee’s paper forms. Find the new online system here. Courses submitted via the new system will be automatically forwarded on for any additional required review upon approval.

Individuals wishing to propose a change to a major or minor (add/revise/delete) should select and complete the appropriate form from below.

  • Forms should be submitted to pamela.bedore@uconn.edu as Word files by a departmental representative to the committee.
  • Supporting documents such as syllabi, plans of study and CVs should be sent as separate documents (either Word or pdf).
